New Hire Information


  • **NEW HIRES CAN ONLY ENROLL AFTER YOUR FIRST DAY OF HIRE**
  • Enrollment sessions last 15-20 minutes – YOU determine the length and pace of your session
  • Dependents’ social security numbers and dates of birth are required to be covered
  • If you are at a computer during your enrollment call, you will have the option to view the Benefit Counselor’s screen during your enrollment
  • Employees who work more than 20hrs per week are required to enroll or waive benefits

Qualifying Life Events / Change of Family Status


Qualifying Life Events MUST be completed within 30 days of your life change
Supporting documentation is required to substantiate a qualifying life event change and must be provided to Human Resources. Supporting documentation includes, but is not limited to:
Marriage License
Birth Certificate
Divorce Decree
Death Certificate
Court Order
Proof of Other Coverage
Proof of Loss of Coverage

To complete a change due to a Qualifying Life Event, click below to login to the benefit enrollment system.

The following information will be required to log into the enrollment platform:

Employee ID or Social Security Number

PIN: Last 4 digits of your Social Security Number and the last 2 digits of your Year of Birth (if last 4 ssn is 1234 & birth year is 1967, pin will be 123467)
