New Hire Information

**NEW HIRES CAN ONLY ENROLL AFTER YOUR FIRST DAY OF HIRE**

Enrollment sessions last 15-20 minutes – YOU determine the length and pace of your session

Dependents’ social security numbers and dates of birth are required to be covered

If you are at a computer during your enrollment call, you will have the option to view the Benefit Counselor’s screen during your enrollment

Employees who work more than 20hrs per week are required to enroll or waive benefits

Qualifying Life Events / Change of Family Status
Qualifying Life Events MUST be completed within 30 days of your life change
Supporting documentation is required to substantiate a qualifying life event change and must be provided to Human Resources. Supporting documentation includes, but is not limited to:
Marriage License
Birth Certificate
Divorce Decree
Death Certificate
Court Order
Proof of Other Coverage
Proof of Loss of Coverage
To complete a change due to a Qualifying Life Event, click below to login to the benefit enrollment system.
The following information will be required to log into the enrollment platform:
Employee ID or Social Security Number
PIN: Last 4 digits of your Social Security Number and the last 2 digits of your Year of Birth (if last 4 ssn is 1234 & birth year is 1967, pin will be 123467)
